Step 4: File A Claim. In you do receive a Workers’ Compensation Claim Form (DWC 1) from your employer, be sure to fill out and sign only the employee portion of the form. Try to describe your injury as completely as possible and include every part of your body affected by the injury. When you have completed the employee portion of the claims form, give the form to your employer. This is called “filing” the claim form.
